Additional hours available by appointment. Occupational Therapy appointments available by request. We also offer outreach to Nashua Plainfield and North Butler High Schools. We provide free screening to injured athletes. PLANNING YOUR FIRST VISIT If you are new to the Nashua clinic, we ask that you print and complete a patient information form and patient medical history form.
