NEW PATIENTS   WELCOME! We ask all our patients to fill out and bring our new patient form to your first visit.   Nashua and Parkersburg have additional forms to fill out that are now available online:  Nashua Clinic  Parkersburg Clinic   TAYLOR PHYSICAL THERAPY CLINIC LOCATIONS   We welcome new patients and will work to schedule your initial evaluation in a timely manner. Following your initial treatment session we will schedule your follow up appointments to work around your schedule making every effort to ensure you follow with your initial provider throughout your treatment. You will be required to fill out a brief medical history and consent for treatment form at your first visit. You can reduce the overall time required on your first visit by downloading and completing your initial visit forms here. Please be sure to select the forms for the clinic location where you will be receiving your treatment. If your doctor gave you a referral/prescription for therapy please be sure to bring that with you to your initial appointment. You will want to bring or dress in loose/comfortable clothing and comfortable footwear to allow you to participate in strengthening, stretching and exercise programs during your treatment.
