To serve our patients in the best way possible, Taylor Physical Therapy has six locations located throughout northeast Iowa. Browse the map below to find the closest clinic location to you and call us to schedule an appointment to speak with a member of our therapy team.

If you're preparing for your first visit to one of the Taylor Physical & Occupational locations, you may download, print, and fill out the following forms. Please bring the documents with you when you come to your appointment.

All of our therapists offer telehealth visits to accommodate scheduling conflicts such as COVID-19 and other health-related issues, inclement weather, and the inability to travel to our clinics! To establish if you are a candidate for telehealth, call our Taylor Physical Therapy office at the Waverly Health Center at 319-352-5644.
